Community nursing is for NDIS participants who need clinical care at home because of their disability. It helps people stay well, avoid unnecessary hospital visits and stay living where they want to live.
Depending on your needs, community nursing can include:
Our team includes Registered Nurse Sara Mansour. We work with your GP and specialists so everyone is on the same page, and our nurse and support workers work together so your day-to-day care stays consistent.
The NDIS funds nursing that relates directly to your disability, usually through Core Supports – Assistance with Daily Life or Capacity Building – Improved Daily Living. General health care, such as treating an illness unrelated to disability, is usually covered by Medicare or the health system. We can help you work out what your plan covers.
We provide community nursing in your home across Melbourne’s northern suburbs.
Do I need a referral from my GP?
Not always, but it helps. A GP letter or hospital discharge summary makes it easier to plan your care.
Can nursing and personal care be provided together?
Yes. Our nurse and support workers can work as one team around your needs.
